You are quite correct regarding medically necessary and/or effective (true for many items if person is using Medicare). Also, one needs to add "experimental" to the list. If Medicare designates something as experimental, most insurance carriers will follow along.
If the employer who negotiates the plan with the insurance carrier wants it included in the prescription coverage, there is a good chance it will be allowed.
