Nationwide, smoking rates range from a high of 29% in Kentucky to a low of 11% in Utah, according to Gallup-Healthways Well-Being Index data collected in the first half of 2011.

3. I was wondering how they got these numbers? If they were

just questionnaires, people may have not answered honestly for a myriad of reasons.

I think "boots on the ground" comments like yours may give a more realistic pic.
