1. Bravo for Jack McCoy!
I will have to watch out for that episode. I must see it.

There was an episode a while back, while McCoy was still an ADA, in which an HMO-type company was placed in charge of medical care for inmates at Rikers. A mentally ill inmate was released with no treatment while incarcerated, was severely psychotic, and murdered a woman (on the subeway, as I recall), and McCoy went after the company for its role in the death, in particular the CEO. It turned out that doctors were demoted or even fired for recommending treatment that cost too much. The CEO was found guilty.
