HIV drugs cause a more pronounced loss of facial fat, which is pretty easily treated with fillers. These fillers would be cheap and accessible in Thailand. People taking HIV drugs are very self-conscious of any facial atrophy and usually do whatever they can to fix this tell-tale sign.

I think he's going for the skinny look, which leads me to believe it's anorexia. I think pedophiles commonly have body image problems because their brains are stunted in adolescence. They really don't want to face the fact that they are adults.

Also, facial thinning is not uncommon in aging, which is why fat grafting is often performed instead of or in conjunction with a facelift these days.

How old is the guy? Has anyone said? I'm guessing that he couldn't be much older than 40-something, at the outside, which would probably rule out significant age-related facial thinning. I'd be more inclined to believe anorexia or disease-related emaciation.
