whittemore institute questionnaire available- fibro/cfs/retrovirus research

the whittemore institute is the medical school that made news recently with the finding that there may be a retrovirus involved in chronic fatigue/fibromyalgia. they now have a questionnaire online for blood donations for their research.

1. Why would I fill out that form, pray tell???

They want your name, address, age, phone number and then for you to disclose information which could make you in-eligible for insurance coverage in the future.
there is no guarantee the information is private.

2. it is the first step in donating tissue for research.

if you already have a dx, and have filed an insurance claim, that cat it out of the bag. even if i don't gain any information, myself, i might help them understand this crap.
i have no idea whether there is a guarantee of privacy, although i guess i assume so. but whatever. i have a medical file that would be a foot thick if you gathered it all up. i am as papered as you can get.
