Anyone remember the already voted story? and showing up dead?

People were showing up to poll places and when they did someone had already voted. I can't remember were I read what state it was happening in. Another thing was that one of the states had people showing up as dead when they showed up to vote.

3. I remember the one where someone came in to vote

and someone else had voted in their name. Might have been in the depositions they took in Ohio about the problems people had trying to vote.
