HELP! I need a scan of the Bush rally loyalty oath.

I've got our local paper interested in doing a story on this, but they asked me for a copy of the oath, and I don't have such a thing. Does anyone remember where online I can get a printable copy of the actual form? I have found many things that give verbatim what it says...and doing google searches on google and here so far have come up dry

in any event, the typos (herby) and nonsense (what is the United States seeking election to?) do speak to its authenticity.
