I used to go up to Canada quite a bit and I'm sure I used my credit cards on numerous occasions.

The form (if you didn't receive a notice letter) asks for your full Visa # and last four of your SSN.

Information on claiming should be included with your credit card statement.

5. It isn't really worth it for the 9$ check..

I did get 11$ when my class action against the RIAA went through. I'm sure that bought a lawyer a nice house in the hills.

I prolly spent $1000 per trip, and made about 10 trips. Just a guesstimate.
