3. Get a lawyer. It's suprisingly standard procedure with insurance companies.
They are in the business to minimize costs. That is the primary function of an insurance company. If dealing with you suddenly becomes more expensive than paying for the operation, they'll pay for the operation.
4. I once saw a NY Times article in which an insurance co. executive ADMITTED
that his company routinely denied all higher-priced claims the first time in hopes that the doctor or patient would get discouraged and not resubmit them.
