In the midst of the national drama over Terri Schiavo, one clear lesson in the legally and emotionally complex case has emerged: Legal experts say all adults should have a durable power of attorney for health care.


Experts say creating a durable power of attorney for health care or a living will can prevent legal wrangling later.


If one's deliberate wishes are spelled out in a legal document -- and if one or more person is appointed to carry out those wishes -- it's less likely that confusion will arise about how to care for that person.


With the help of legal experts and templates from the State Bar of Michigan, the Free Press is providing readers forms in today's paper. These forms can be used by those wanting to complete a living will or power of attorney form, but are not meant to replace an attorney's advice.

3. Be careful! Each state has different living wills....
Some states require notarization...and others have more stringent rules for witnesses. Check out the form in your own state...they're available on the web. The one here is the Michigan form....and may NOT be recognized in other states.
