If you have a 1 in a million shot of getting some disease and 500 million people are vaccinated that means that 500 people who were vaccinated will get that disease, of those there is a good shot one will be diagnosed around the same time(like say if they're already in the doctors office for a shot and they happen to mention some weird symptoms they've had, and the doctor does some routine lab work).

Unfortunately to a great many people this extremely weak correlation proves causation.
