I don't know that I agree with the assessment that the hole is "not a small man made hole, it's too big for that." From that picture, it is difficult to tell the shape and size of the hole.

But, based on the amount of smoke in the picture, it does look like the tower has been burning for enough time to have filled those floors with smoke. My first reaction is that the "hole" was made by some of the people trapped in the tower breaking out windows desperately trying to get some air to breathe.

Perhaps someone can find a better picture of this and allow us to get a better idea of what is was.
