In some people it causes symptoms mistaken for colon cancer, colitis, and all kinds of nasty stuff.

If he is worried enough to go, he should, but don't panic about it. Most of the serious diseases you are worrying about would not cause any lower gut ache until a lot of other symptoms were pretty advanced and impossible to ignore, too. So it's probably diet/stress/aging related. Don't take this as me advising against getting the exam, just me telling you not to be scared (I know that's easier said than done). Most of the time it's not what you fear, so the exam will take a load off your mind. Good luck to both of you.
