The dock of my iMac has a 2nd FireFox icon that says "Firefox from iPhone4" when I don't have one

I don't have nor have I ever had an iPhone 4.

I tried rebooting, it went away, but then came back.

Not gonna lie, I am a little freaked out

Go to System Settings > Apple ID

Then have a look at the devices list. If there are any devices you do not recognise then select it and remove from your account.
